Home Health Business Development Liaison

As a Home Health Business Development Liaison, you'll be the face of Choice Health At Home in the community — developing strong relationships with physicians, hospitals, case managers, senior living communities, and other referral partners. Your work will directly help patients receive quality care in the comfort of their homes while contributing to the continued growth and success of our organization.

What You'll Do

Build and strengthen referral relationships to position Choice Health At Home as the provider of choice
Develop new business opportunities through strategic community outreach and networking
Collaborate with clinical and operational teams to ensure smooth patient transitions and exceptional care experiences
Promote awareness of Choice Health At Home services within the healthcare community
Meet and exceed monthly referral and admission goals
Maintain knowledge of industry regulations and ensure compliant marketing practices
Help identify community needs and contribute to program growth initiatives

What We're Looking For

2+ years of home health or hospice sales experience preferred
Strong relationship-building and communication skills
Experience with CRM software preferred
HomeCare HomeBase experience preferred
Valid Driver's License and Auto Insurance
