Deliver exceptional patient care in nursing home and assisted living settings, providing essential services for insurance members residing in skilled facilities. This contract position focuses on delivering comprehensive healthcare with an emphasis on treating patients in place, decreasing unnecessary hospitalizations, and supporting successful transitions to home environments.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• No weekends or holidays required

Key Responsibilities:

• Conduct regular rounds across several facilities to ensure ongoing clinical oversight and patient well-being
• Complete routine monthly health assessments and additional visits as needed, following directives from the Nurse Practitioner
• Monitor and document residents progress, assisting them in meeting care goals
• Attend interdisciplinary team meetings on behalf of the Nurse Practitioner, relaying critical updates to care partners
• Perform comprehensive care coordination activities, collaborating with multiple healthcare professionals to optimize patient outcomes

Qualifications:

• Current NY RN license
• Bachelors or Associates degree in Nursing (BSN or ASN)
• Basic Life Support (BLS) certification
• Previous experience in long-term care or similar settings preferred
• Strong communication skills with an ability to navigate multidisciplinary team environments

Compensation: Earn between $65 and $80 per hour in this impactful role dedicated to improving healthcare delivery and avoiding preventable emergency visits and hospital stays.
