Physical Therapist - Outpatient

Employee type: Regular. Work shift: Day, night, or flexible as needed. Position Summary Responsible for providing a full range of therapeutic services including assessment, treatment planning, and therapeutic interventions in an interdisciplinary environment consistent with professional practices and ethical standards. A physical therapist (PT) must evaluate a patient’s condition, devise a customized rehabilitative and treatment plan, and work to develop, maintain, and restore maximum movement and functional ability to maximize quality of life in an outpatient rehab setting. Essential Functions Review physician's referral, patient’s condition, medical history, and submitted medical records to determine physical therapy treatment required. Plan and prepare written treatment program based on patient evaluation, incorporating proper interventions to help patients improve everyday tasks, decrease pain, ambulation, and enhance strength and mobility. Evaluate patient’s functional ability using standardized tests, measuring strength, motor development, sensory perception, functional capacity, and level of independence with daily activities, and record findings. Develop treatment plans with specific goals and tasks. Administer treatments involving physical agents such as cold packs, contrast baths, moist packs, ultraviolet and infrared lamps, iontophoresis, electrical stimulation, and ultrasound machines. Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit. Record treatment, responses, and progress in the patient’s electronic medical record and prepare reports as required by licensure, providers, or insurance recommendations. Instruct patient and family in home treatment procedures. Complete documentation per clinical / department guidelines and meet expectations of monthly department chart audits. Non‑Essential/Marginal Functions Complete required orientation as directed by facility. Follow facility and OSHA safety rules and procedures while on assignment. Follow facility and Joint Commission guidelines and recommendations. Respect cultural and religious practices of patients. Uphold HIPAA regulations. Be punctual and dependable for assigned/confirmed shifts. Promote Tidelands Health Rehabilitation programs to referring physicians, healthcare providers, and/or co‑workers by performing in‑services and/or marketing activities. Proactively impact cancel/no‑show percentage to improve quality care and patient outcomes. Achieve established average monthly patient care productivity metric. Requirements and Qualifications Must be a graduate of an accredited therapy program and have completed required clinical internships satisfactorily. Licensed by the State of South Carolina Board of Physical Therapy Examiners. Must be able to utilize therapy equipment in a Tidelands Health rehabilitation setting to provide treatment and procedures. Functional supervision of physical therapy assistants and rehab technicians. Physical Requirements Moderate physical demands: frequently required to stand, sit, walk, use hands and fingers, talk or hear.
