RN Home Health - Cambridge, Massachusetts

Deliver compassionate, skilled nursing care in a home health setting, supporting patients from infancy through advanced age with individualized clinical services. This contract opportunity involves conducting home visits throughout the Cambridge area, where you'll assess patient needs, implement evidence-based care plans, and empower patients and families with education and ongoing support. Your case management expertise will ensure comprehensive admissions and coordinated care in line with best practices and established protocols.


Schedule/Hours:

• Weekend rotations required (once every 6 weeks)
• Must be willing to work holidays and participate in on-call duties as scheduled

Qualifications and Desired Experience:

• Current, active Massachusetts RN licensure
• Minimum of 1 year experience in certified home care nursing
• At least 1 year of direct case management experience
• Comfortable working independently and efficiently in a home care environment
• Reliable and independent transportation for client visits
• Commitment to accommodating weekend, holiday, and on-call scheduling needs

Key Responsibilities:

• Deliver a full range of clinical services to patients in their homes, tailored to diverse ages and health needs
• Assess patient health, document observations, and update care plans using electronic documentation systems
• Manage the admissions process and ongoing case management per established standards
• Educate patients and families to promote self-care, safety, and positive health outcomes
• Collaborate across interdisciplinary teams to coordinate care and ensure continuity
• Provide prompt and accurate communications regarding patient status and plan adjustments
