Job Description A licensed physical therapist who meets the Code of Ethics/Standards of Practice as outlined by their professional organization and by the State of Illinois. The position includes direct patient care, teaching, participation on appropriate committees, and departmental process improvement. Serves as a role model for all therapists and clinical interns in all aspects of professional, clinical, communication, education, and leadership activities. Responsibilities Demonstrates the four NM values of teamwork, patients first, integrity, and excellence in daily work and interactions. Provides accurate information and timely updates to patients and customers; addresses questions and concerns promptly or identifies appropriate resources. Maintains primary responsibility over patient plan of care through timely and consistent collaboration with treating PT/PTA. Demonstrates teamwork by helping co‑workers both within and across departments; communicates effectively, respects diverse opinions, and acknowledges others’ contributions. Uses organizational and unit/department resources efficiently. Manages work schedule efficiently, completing tasks and assignments on time. Contributes to continuous improvement opportunities and processes. Participates in efforts to reduce costs, streamline work processes, and improve services. Demonstrates creative problem‑solving and adaptability in treatment plans, evidenced by team feedback and documentation. Exhibits essential skill competencies in evaluating treatment modalities, equipment, and protocols; performs re‑evaluations per department guidelines and adjusts the plan accordingly. Assumes full responsibility for patient management from initial contact to discharge, ensuring functional progress, safety, and treatment outcomes. Adheres to department policy regarding evaluations, progress notes, and discharge summaries; documents all notes in electronic medical record (EMR). Completes daily notes that reflect the patient’s response to treatment, actual performance, and assessment status, meeting all documentation requirements. Provides educational materials appropriate to the patient’s level of understanding and motivation. Participates in committees, department processes, and special projects in accordance with clinical expertise and individual performance plan. Builds professional relationships with healthcare teams (e.g., physician, nurse, case manager) and participates in internal and external marketing as expected. Creates a culture of respectful communication by listening, soliciting input, and acknowledging contributions. Accepts constructive feedback and makes timely changes, evidenced by coaching forms, chart audits, and team feedback. Proactively completes non‑patient care responsibilities within established timelines (coverage notes, PTO/schedule change requests, bonus pay forms, follow‑up calls). Contributes to departmental educational efforts (e.g., in‑services, journal clubs, new hire orientation). Actively participates in student clinical or observation programs as a clinical instructor, mentor, or host as required. Maintains productivity standards as expected by the department. Demonstrates willingness to flex schedule or assignment to meet patient care needs and attend necessary meetings outside normal hours. Qualifications Required Bachelor’s Degree in Physical Therapy. Licensed Physical Therapist in the State of Illinois. BLS certification from the American Heart Association upon hire. Preferred Master’s or Doctoral Degree in Physical Therapy.
