Patient Care Coordinator

Job Summary


Our client is seeking a Care Navigator to manage outbound referrals efficiently. The primary responsibilities include working through queues to ensure patient follow-ups, tracking specialist appointments, and gathering necessary consult notes.


Key Responsibilities

• Verify whether patients have attended their specialist appointments.
• Document cases where patients refuse and send notes to their providers.
• Contact specialists to obtain consult notes and results for attended appointments.

Qualifications

• Referral Experience or Clinical Experience required.
• Preferred: Experience with e-clinical works or other EMR systems.

Skills

• Proficient in managing clinical referrals and outbound communications.
• Experience with electronic medical records preferred.

Additional Requirements

• Monday-Friday, 8 AM to 5 PM schedule.
