LTC Social Worker: Discharge & Resource Specialist

SEARHC is seeking a qualified Social Worker to join the Long-Term Care interdisciplinary team in Sitka, Alaska. The role advocates for LTC residents and families, conducts psycho-social assessments, and coordinates discharge planning across inpatient, LTC, and outpatient settings with attention to payer resources. You will collaborate with physicians, nurses, and social services to develop care plans, assist with Medicaid/third-party enrollment, and ensure documentation meets regulatory #J-18808-Ljbffr SEARHC
