Your responsibilities include:Clinical Leadership & OversightProviding overall medical direction to the program, including supervision of all hospice providersConsulting with attending physicians on pain and symptom management for hospice patientsActing as the primary medical resource to the hospice interdisciplinary groupEnsuring competent physician services are available on a 24-hour basis to meet the general medical needs of hospice patientsReviewing and developing treatment protocols and proposing current, evidence-based intervention optionsMaintaining current knowledge of the latest research and trends in hospice, palliative care, and pain/symptom managementServing as collaborating physician for practitioners working within the organization, as neededQuality, Policy & ComplianceEstablishing and continually reviewing policies and procedures related to patient care, medical education, and emergency protocolsEnsuring adherence to Medicare Conditions of Participation, CHAP standards, and applicable federal and state statutesEvaluating and monitoring QAPI plans to identify medical education needs; participating in QAPI teams and activitiesDeveloping criteria — in partnership with the COO and VP of Operations — to monitor the quality of physician and mid-level provider education programsAssisting with evaluation of protocols and procedures with respect to quality and cost outcomesAssuring that organizational policies, bylaws, rules, and regulations are followed throughout the programEducation & Community EngagementActing as a liaison to physicians by providing consultation and education to IDG physicians, attending physicians, and community physicians related to hospiceServing as a hospice champion — representing and promoting the program to physicians, discharge planners, referral sources, and community health organizationsProposing and supporting organizational programs that address identified medical education needsDemonstrating skill in counseling and communicating with patients and families navigating end-of-life careParticipating in resolution of interpersonal conflict and issues of clinical and ethical concernEngaging in ongoing medical education activities related to hospice and palliative careBenefits We OfferCompetitive payCompany provided equipmentWhy Choose Transitions Hospice?Hospice physicians at Transitions aren't just consultants — they're leaders, educators, and advocates for a better end-of-life experience.
