We are looking for a Home Health Case Manager to join our Home Health Physical Therapy Team in Franklin, NC . This role is essential in ensuring that we maintain high-quality, efficient care for all patients. Job Summary and QualificationsIn this role, you will collaborate with colleagues in the Care Management department, service line, and interdisciplinary care team to ensure quality-driven best practices throughout the hospitalization continuum. This involves utilizing the eight essential activities of case management, including assessment, planning, implementation, coordination, monitoring, evaluation, outcomes, and cost containment. You will also work to maximize positive financial outcomes for designated patient populations by intervening at critical points in the disease process, conducting post-discharge follow-up calls, and maintaining crucial communication with care providers to promote health compliance and prevent readmissions. Additionally, you will provide leadership functions within the Care Management Department.
Job Summary:
You will provide and oversee Physical Therapy services for patients, which includes assessment, treatment, program planning, and implementation. This role operates under physician orders and adheres to the principles and practices of physical therapy, organizational policies, and state regulations.
Key Responsibilities:
- Enhance patients' physical abilities by performing or supervising therapy staff in initiating referrals, evaluating and treating patients, participating in care planning, documenting progress, and communicating patient status to relevant parties.
- Establish individual patient schedules in collaboration with team members. Develop treatment plans that identify problems, set long- and short-term goals, and outline methods to achieve these goals.
- Coordinate treatment plans with patients, families, and team members. Implement and conduct treatments as specified in the treatment plan, monitor patient responses, and adjust treatments as necessary to meet goals in accordance with physician orders. Document all assessments, treatments, follow-ups, and service terminations.
- Create appropriate home programs to maintain and enhance patient performance in their environment. Participate in discharge planning and provide recommendations for adaptive devices, orthotics, or prosthetics, training patients and families in their use.
- Refer patients to Physical Therapy Assistants (PTA) by completing evaluations and initial treatment plans, ensuring effective communication of necessary information. Supervise and guide PTA in executing the treatment plan.
- Review the quality and appropriateness of services delivered and individual physical therapy programs for effectiveness and efficiency using established criteria.
- Engage in patient, family, and staff education. Collaborate formally and informally with team members to coordinate the total rehabilitation program for patients. Attend required in-service training and facility meetings.
- Submit billing, payroll, and expense documentation accurately and promptly. Assist in collecting statistics as directed.
- Adhere to all Infection Control, Standard Precautions, and OSHA standards for healthcare professionals. Participate in Continuous Quality Improvement Programs as required and maintain confidentiality standards regarding patient information.
- Demonstrate sound judgment consistently in evaluating, planning, implementing, and following up on patient therapy programs.
Education and Experience: Bachelor of Science in Physical Therapy from an accredited institution.
Certifications, Licenses, and Special Requirements: Completion of the National Certification Examination for Registered Physical Therapists. A valid license to practice physical therapy in the state is required, along with a valid NC Driver's License.

- Flexibility to work varying hours, including weekends and holidays.
- Bilingual candidates who can read, write, and speak Spanish fluently are preferred.
