Case Manager (Children's Community Action Team)

New Horizons is a nonprofit mental health and addiction recovery provider on the Treasure Coast, serving children and adults throughout Indian River, Martin, Okeechobee and St. Lucie counties. We offer crisis services, a 24‑hour help line, mobile response and outpatient programs. Case Manager Responsibilities Assist other team members with Crisis Intervention Services for the client and/or family. Develop an individualized service plan for each client based on assessment information, outlining long‑term desired outcomes and comprehensive strategies. Advocate and support clients assigned to the CAT Team, connecting them to appropriate services and monitoring progress. Serve as agency liaison with DCF, DJJ and other community agencies for referral coordination and community presentations. Assist in transporting youth or families to medical appointments, court hearings or other activities outlined in the Plan of Care. Qualifications Bachelor’s degree in social behavior science or a related field. Three (3) years of applicable experience. Valid Florida driver’s licence with 6 points or less in the last 5 years.
