What You'll Do
- Provide skilled physical therapy services to patients in their homes
- Assess patient needs related to wounds, heart failure, diabetes, and IV therapies
- Develop and implement individualized treatment plans
- Educate patients and caregivers on therapies and health management
- Document patient progress in Meditech Home Care EMR
- Collaborate with healthcare team members across different locations within a 50-mile radius
- Support within the department as an educator and resource
What You Need
- Required:
- Valid Louisiana licensure or equivalent
- BLS certification
- 1+ year of relevant acute care experience
- Preferred:
- Experience with EMR systems (Meditech Home Care preferred)
- ICU or ER experience a plus
- Strong communication and patient education skills
