Candidate Must Have Have a Master's degree from a school accredited by the Council on Social Work Education Has a bachelor's degree in social work, psychology, sociology or other field related to social work One year of Home Health experience Ability to exercise initiative and independent judgment Must be a team player Level 2 Background screening Responsibilities Serves as a resource person to employees, patients, physicians and other allied healthcare providers Participates in the development of the plan of care Prepares clinical and progress notes Assists physician and other team members in understanding the significant social and emotional factors related to health problems Works with the family Participates in discharge planning Documents all patient/family services provided as required by Agency policy Plans/coordinates all social services within the Agency Maintains contact with community support groups and provides professional expertise as required Maintains current knowledge of Medicare Conditions of Participation for social work services Immediately reports any accident, incident, lost articles or unusual occurrence to Director of Clinical Services/Nursing Supervisor Participates in patient care conferences Participates in the coordination of Agency's services with the services of other community resources; uses community resources Serves as a resource person to employees, patients, physicians and other allied healthcare providers Other duties assigned by Director of Clinical Services/Nursing Supervisor This is a Per Diem position Job Types: Full-time, Part-time Pay: $85.00 per hour Schedule: Choose your own hours Day shift Experience: MSW in Home Health: 1 year (Required) Work Location: On the road For additional information, visit our website Acutehomeheathcare.com . #J-18808-Ljbffr Acute Home Healthcare
