Nurse Practitioner – Family Medicine

Location: San Jose, CA
Salary: $158,000–$185,000/year | Full-Time

About the Role:
Facility seeking a compassionate Family Nurse Practitioner to provide primary care services to underserved communities in a mission-driven FQHC setting.

Key Responsibilities:

• Provide primary care, diagnosis, treatment, and preventive services.
• Conduct exams, develop treatment plans, and manage medications.
• Educate patients and monitor outcomes.
• Collaborate with physicians and multidisciplinary teams.
• Maintain accurate EHR documentation and coordinate referrals.

Requirements:

• Master’s or Doctorate from an accredited NP program.
• Active, unrestricted California NP license .
• Board Certified.
• Active, unrestricted DEA license .
• Current BLS certification .
• 2+ years of primary care experience required.
• FQHC/community health experience preferred.
