We are hiring a Full-Time Family Nurse Practitioner (FNP) for a primary care clinic in San Jose, CA . Required Qualifications

• Master's or Doctorate from an accredited Nurse Practitioner program
• Active California Nurse Practitioner License
• Board Certified by the California Board of Registered Nursing
• Active, unrestricted DEA License
• Current BLS Certification
• Minimum 2 years of Primary Care clinical experience
• Community Health Center or FQHC experience preferred

Responsibilities

• Provide comprehensive primary care, including assessments, diagnosis, and treatment
• Perform physical exams and obtain medical histories
• Prescribe and manage medications
• Develop individualized treatment plans
• Educate patients on disease prevention and wellness
• Coordinate referrals to specialists
• Collaborate with physicians and interdisciplinary teams
• Maintain accurate EHR documentation
• Participate in team meetings and after-hours on-call rotation
