Community Health Worker Department: Population Health Employment Type: Full Time Location: 19500 HWY 249, Suite 570 Houston, TX 77070 Reporting To: Obiageri Adeojo Compensation: $23.00 - $28.00 / hour Description About the Role Provide a link between patients and available resources to provide the necessary resources in a cost-effective manner. Our Values Put Patients First Empower Entrepreneurial Provider and Care Teams Operate with Integrity & Excellence Be Innovative Work As One Team What You'll Do Provide home visits to risk patients and families for evaluation of needs in an effort to provide direction of care for desired outcomes in a cost-effective manner. Report specific health & social information back to the care management team to asset in development of care pathways. Participate in multidisciplinary team conferences as needed. Assist patients with accessing needed services, recognize potentially serious problems to prevent poor health and social outcomes. Qualifications High school diploma or equivalent. Minimum of 1 year of experience working with a community agency providing health care services, including community resources, communication skills, individual and family advocacy, health education, and service skills. Active patient of the state's Community Health Worker Association. Effective communication skills. Extensive experience with Microsoft Office Suite (Word, PowerPoint, Excel). Professional work ethic. Strong comfort with working in a fast-paced environment. An authentic passion for improving the lives of underserved patients and advancing value-based care. Licenses/Certifications Community Health Worker (CHW) certification through a college-level program. State-Specific Requirements Ohio Plan: CHW certification through a college-level program or LPN or LCSW license required. Valid driver’s license required. Michigan Plan: CHW certification preferred but not required. Valid driver’s license required. Illinois Plan: CHW certification preferred but not required. Valid driver’s license required. Medicare: CHW certification through a college-level program or LPN license required. Valid driver’s license required. Kentucky Plan: CHW certification preferred but not required. Valid driver’s license required. Environmental Job Requirements and Working Conditions Our organization follows a hybrid work structure where the expectation is to work both in office and at home on a weekly basis. The office is located at 19500 HWY 249, Suite 570 Houston, TX 77070. The total compensation target pay range for this role is: $23 - $28 per hour.
