Social Worker Full-Time | Skilled Nursing / Rehabilitation Salary: Up to $80,000 per year The Riverside Premier Rehabilitation and Healing Center is seeking a dedicated and compassionate Social Worker to join our interdisciplinary team. About Us Our facility offers a dynamic clinical setting with a strong focus on short-term rehabilitation and long-term care. With a large bed capacity and a state-of-the-art rehab program, we provide services seven days a week and foster a collaborative environment that supports both professional growth and exceptional patient outcomes. Social Worker Position Overview The Social Worker plays a key role in supporting residents and their families by addressing psychosocial needs, facilitating care planning, and ensuring safe and effective discharge transitions. Social Worker Key Responsibilities Assess residents’ social, emotional, and psychosocial needs Develop and implement individualized care plans Participate in interdisciplinary care plan meetings Coordinate admissions, transfers, and discharge planning Assist with securing home care services, durable medical equipment (DME), and community resources Provide support and guidance to residents and families throughout their stay Collaborate with clinical and administrative teams to ensure continuity of care Assist with financial documentation and resident education as needed Advocate for resident rights and quality of life Social Worker Qualifications Bachelor’s Degree in Social Work (BSW), Master’s Degree (MSW), or related field preferred Prior experience in a skilled nursing facility, rehabilitation center, or long-term care setting preferred Strong communication, organizational, and interpersonal skills Ability to manage multiple priorities in a fast-paced environment Knowledge of discharge planning, community resources, and care coordination Familiarity with PCC and MDS 3.0 is a plus Compensation & Benefits Salary up to $80,000 annually (based on experience) Medical, Dental, and Vision Insurance Paid Time Off 401(k) Retirement Plan Employee Perks & Discount Programs Opportunities for professional growth and advancement The Riverside Premier Rehabilitation and Healing Center is proud to be an Equal Opportunity Employer. #boost #J-18808-Ljbffr
