Hospice Social Worker — Impactful End-of-Life Care

VIA Health Partners in Gastonia, NC, seeks a licensed social worker to deliver comprehensive end-of-life care. You’ll conduct assessments, develop care plans, and work closely with patients and their families, ensuring culturally sensitive care. Ideal candidates should have a Bachelor's or Master's degree in social work, and experience in hospice care is preferred.
