Preferred Qualifications Role Purpose Provides individual attention to each patient and needed support by navigating patients through the complexities of the screening, diagnostic and treatment trajectory. Responsibilities Navigates customers through the screening, diagnostic and treatment trajectory. Tracks interventions and outcomes. Completes and processes all necessary paperwork and documentation to enroll patients in appropriate programs. Facilitates communication and education with health care staff and providers.
