Physical Therapist - Travel Contract

Physical Therapist — Outpatient | Greenville, OH

Provide outpatient physical therapy services in Greenville, OH. Conduct 60-minute evaluations and develop individualized treatment plans aligned with patient goals and physician direction. Deliver discipline-specific therapy, delegate tasks appropriately, and educate patients, families, students, and coworkers. Complete timely and accurate documentation including goal writing, functional quality indicators, letters of medical necessity, and charge entry in accordance with clinic policies and regulatory guidelines. Maintain professional licensure and adhere to state practice acts and organizational standards of conduct. Participate in performance improvement activities and contribute to a safe work environment through adherence to safety policies and safe patient handling practices.

Minimum Qualifications

• Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Ohio physical therapy license required to start the assignment.
• BLS certification from the American Heart Association or American Red Cross may be required.
• Experience providing outpatient physical therapy evaluations and treatment, including 60-minute initial evaluations.
• Ability to complete documentation in compliance with state, federal, and payer guidelines.
• Familiarity with safe patient handling, infection control, and clinic safety protocols.

Preferred Qualifications

• Experience supervising students and participating in clinical education.
• Previous experience working in an outpatient clinic setting with functional, community-focused treatment programs.

Assignment Details

• Schedule: 5x8 shift pattern.
