LVN / LPN - LTC/SNF

Call for Salary Rate -

Job Summary:

Provide high-quality nursing care to residents in a rural LTC/SNF setting, ensuring safety and well-being while adhering to all regulatory standards. Collaborate with the healthcare team to deliver comprehensive resident care and maintain effective communication with residents' families.

Responsibilities:

• 
  Observe and document residents' mental and physical status, reporting significant changes to the Supervisor.
  
  
• 
  Administer medications, treatments, and oxygen in accordance with physicians orders and facility protocols.
  
  
• 
  Supervise and assist Licensed Nursing Assistants (LNAs) to ensure optimal resident care within scope of practice.
  
  
• 
  Participate in resident rounds with physicians, addressing medical needs and facilitating communication during reviews.
  
  
• 
  Complete all required documentation accurately within electronic systems, including MARs/TARs and nursing notes.
  
  
• 
  Manage unit operations by checking appointments, monitoring therapy schedules, and ensuring infection control practices are followed.
  
  

Qualifications:

• 
  Valid LPN/VN license active at the time of employment.
  
  
• 
  Minimum of 3 years recent nursing experience, preferably in LTC/SNF or rural settings.
  
  
• 
  Current BLS certification; IV certification preferred (willing to obtain if not held).
  
  
• 
  Ability to work independently and supervise staff effectively.
  
  
• 
  Strong communication and documentation skills.
  
  
• 
  Willingness to work in a rural location with reliable transportation.
