Join our travel Physical Therapist team providing in‑home care in Longwood, FL. Provide skilled physical therapy to patients recovering from injury or managing chronic conditions while experiencing life in the Longwood area. Assignment Details: Facility Type: Home Health Shift Name: Days Shift Details: Monday–Friday, 08:00–16:00 Weekly Hours: 40 (full‑time caseload) On‑call/Weekend Requirement: No weekend requirement Mileage reimbursement between visits is provided Qualifications Doctoral, Master’s, or Bachelor’s Degree in Physical Therapy from an accredited program approved by the APTA. Active Florida Physical Therapy license required. Proficiency with HCHB and OASIS required. Experience conducting comprehensive in‑home physical therapy evaluations and developing individualized Plans of Care aligned with physician orders and payer requirements, including Medicare and VA standards. Experience delivering skilled interventions for mobility, gait, balance, strength, endurance, and pain management in the home health setting. Ability to perform home safety assessments and recommend environmental modifications. Experience maintaining accurate electronic medical record documentation (evaluations, progress notes, reassessments, discharge summaries). Ability to manage assigned caseload independently while meeting productivity and visit timeliness expectations. BLS certification may be required from AHA or ARC.
