CLINICAL PHARMACIST

Summary The Clinical Pharmacist provides distributive and clinical pharmacy support services to medicine‑surgery and specialty services within VAWNYHS. The scope of practice includes direct and indirect patient care, drug therapy monitoring, quality assurance/improvement, staff/patient education, and consultation. Qualifications Citizenship: Citizen of the United States (noncitizens may be considered under certain conditions). Education: Graduate of an Accredited Council for Pharmacy Education (ACPE) accredited college or school of pharmacy with a bachelor's degree in pharmacy or a Doctor of Pharmacy (Pharm.D.) degree. Foreign graduates must provide FPGEC certification. Licensure: Full, current, and unrestricted license to practice pharmacy in a U.S. state, territory, Commonwealth, or the District of Columbia. Licensure must be maintained and any revocation or suspension must be fully restored. Physical Requirements: Meet VA Directive and Handbook 5019 standards. English Language Proficiency: Proficient in spoken and written English as required by federal regulations. Experience or Education: At least one year of experience equivalent to the next lower grade level or completion of an ACPE‑accredited Pharm.D. program. Assignment: Handles routine medication‑related activities in accordance with local, VISN, and national policies and regulations, including reviewing, interpreting, and verifying medication orders, processing and filling orders, interacting with clinical staff, reviewing patient information, making recommendations, documenting interventions, providing refill extensions, taking medication histories, performing reconciliation, providing drug information, assisting in formulary management, documenting adverse drug events, assisting in emergencies, and overseeing technical staff. Knowledge, Skills, and Abilities (KSAs): Knowledge of professional pharmacy practice; ability to communicate orally and in writing; knowledge of laws, regulations, and accreditation standards; skill in monitoring and assessing drug therapy outcomes. Duties Execute medication orders, including correct drug selection, dosage form, compounding, packaging, and labeling. Dispense drugs, especially controlled substances, investigational, and cytotoxic medications, ensuring legal and regulatory compliance and proper recordkeeping. Review patient medication profiles prior to dispensing to ensure safe and appropriate use. Identify and manage drug‑induced problems, communicating findings to primary providers. Recommend therapeutic care plans that are effective, least toxic, and economical, considering patient age and life stage.
