Patient Connect Nurse Practitioner (Per Assessment) - York County, ME

$5,000 Sign-on Bonus for External Candidates Optum Home & Community Care, part of UnitedHealth Group, provides integrated care for patients in nursing homes and assisted living. As a member of the Senior Community Care product team, you will support patients during transitions from skilled rehab to home settings, ensuring smooth care transitions and preventing readmissions. Primary Responsibilities Perform comprehensive assessments and document findings in a concise and compliant manner per CMS regulations. Ensure all diagnoses are accurately coded in ICD‑10 and fully documented. Address all required quality elements during each visit. Conduct an initial medication review. Complete mandatory educational and virtual training requirements. Travel between care sites as required. Care Coordination: Communicate medical concerns with the patient and relevant team members during visits. Program Enhancement Expected Behaviors: Develop innovative approaches to care improvement. Work independently with minimal supervision. Submit available hours to the web-based scheduling system at least one month in advance. Demonstrate initiative toward individual, team, and organizational goals. Participate in quality initiatives for the SCC program. Check Optum email intermittently for required trainings, communications, and scheduling. Required Qualifications Certified Nurse Practitioner through a national board. Graduate of an accredited master’s nursing program (MSN) with board certification through AANP or ANCC (Adult‑Gerontology Acute Care, Adult/Family or Gerontology). Preferred certifications: ANP, FNP, or GNP. Active and unrestricted license in the State of Maine. Current DEA licensure or prescriptive authority, or ability to obtain per state regulations. Availability to work at least 10 hours per week. Driver’s license and reliable transportation to travel to patient sites. Preferred Qualifications 1+ years of post‑graduate experience in long‑term care. Ability to lift a 30‑pound bag and navigate various dwelling configurations. Knowledge of geriatrics, chronic illness, and advanced illness/end‑of‑life discussions. Proficient computer skills for documenting in electronic medical records. Proven ability to develop and maintain positive customer relationships. Hourly wage: $43.17 to $84.95 per hour for full‑time employment. Pay is based on local labor markets, education, experience, and certifications.
