Employee Separation Policy: Any past employee of UPH whether FTE, or PRN for the UPH float pool, cannot work travel in another Market no matter which facility they currently work at, or have worked at within the past 12 months. If they worked travel at UPH (not FTE or PRN), the separation policy does not apply

HOLIDAY update: unless the Unit is closed, your contractor is required to work 1-2 of 3 upcoming Summer holidays (Memorial Day, Independence Day, Labor Day)

• NBO: 16 hrs
• EMR : EPIC
• OT/Holiday: 1.15x multiplier
• Call Back: 1.5x multiplier with 2 hr CB minimum
• On Call: ***/hr
• Charge/Precept : ***/hr

Billable OT:

• 36-40 hrs/week, OT billable after 40
• 48 hrs/week, OT billable after 48

Unit Information:

• Beds: 6
• Ratio: 2:1, some 1:1
• Equipment: Fresenius
• No drips/titrates
• Scrub color: any color tops and bottoms, solids

• At least 12 months of Dialysis experience [required]
• 1st time travelers welcomed with at least 15 mos of Dialysis experience
• EPIC electronic medical systems experience [ required ]
• Equipment experience with : Fresenius [required]

• Work history/resume/employment history must have 12 months of current Dialysis experience [required]
