Collaborate with families, providers, and community agencies to support treatment goals and address barriers to progress. Provide families with education, resources, and support to strengthen engagement and improve outcomes for children. Maintain accurate, timely documentation and ensure compliance with agency, Medicaid, and regulatory requirements. Monitor treatment progress, facilitate ongoing review of care plans, and support continuity of services throughout the child's treatment journey.
