Location: Brisbane, Daly City, Colma, South San Francisco, Pacifica and neighboring cities

Assignment Duration

This is a 13-week contract position.

Schedule

Days: 8am - 5:00pm (Schedule can be flexible based on patient's availability.)

Job Description

The Physical Therapist's primary goal is to restore function to prevent or minimize disability following disease or injury, as well as to help clients reach their maximum performance/function within the limits of their capabilities. They will evaluate and treat patients using medically prescribed physical therapy programs. The Physical Therapist may also supervise Physical Therapy Assistants, conduct initial admission/evaluation visits, and notify the assigned Clinical Team Manager of OASIS scores within established timelines.

Responsibilities

• Evaluate and treat patients according to medically prescribed physical therapy programs.
• Document observations, clinical findings, problems, skilled interventions/treatments, goals, and discharge plans.
• Supervise Physical Therapy Assistants as needed.
• Conduct initial admission/evaluation visits and communicate OASIS scores to the Clinical Team Manager.
• Perform other duties as assigned consistent with skills and training.

Required Experience / Certifications / Licensure

One year of Home Health experience is required.
