Wise Path Recovery Centers is seeking an experienced, compassionate Senior Case Manager to support individuals navigating substance use disorders, behavioral health needs, and complex psychosocial challenges.

The Senior Case Manager plays an important role in connecting every aspect of a client’s care. Working closely with our clinical, medical, recovery support, and community partners, this position helps ensure clients have the resources, advocacy, and coordinated support they need throughout treatment and as they transition into the next phase of recovery.

What You’ll Do

As the Senior Case Manager, you will support clients with moderate to complex needs while coordinating services across treatment, healthcare, recovery, and community systems.

Key responsibilities include:

• Conduct biopsychosocial assessments and contribute to individualized treatment planning and ongoing plan updates.
• Provide comprehensive case management for clients with moderate to high psychosocial, behavioral health, and recovery support needs.
• Coordinate care across clinical, medical, psychiatric, recovery support, and community-based services.
• Identify barriers to successful treatment and recovery and collaborate with clients and the interdisciplinary team to develop practical solutions.
• Coordinate discharge and transition planning beginning early in the treatment process to support continuity of care.
• Assist clients with accessing housing, transportation, employment, benefits, healthcare, legal resources, recovery supports, and other community services.
• Facilitate and participate in interdisciplinary case reviews, communicating client progress, barriers, risks, and recommended next steps.
• Coordinate services during crisis situations and assist with appropriate referrals and follow-up.
• Develop and maintain strong working relationships with community agencies, courts, healthcare providers, recovery organizations, and referral partners throughout Southern West Virginia and surrounding communities.
• Advocate for clients while encouraging independence, personal responsibility, and self-advocacy.
• Maintain appropriate professional and therapeutic boundaries at all times.
• Maintain timely, complete, and accurate documentation within the electronic health record.
• Support quality improvement, performance improvement, compliance, and accreditation initiatives.
• Serve as a resource to other team members regarding community resources, complex case coordination, and discharge planning.
• Maintain client confidentiality and comply with HIPAA, 42 CFR Part 2, organizational policies, and all applicable regulatory requirements.
• Perform other duties as assigned.

Requirements

Required

• Bachelor’s degree in social work, psychology, counseling, human services, or a related field; or at least three years of behavioral health experience with demonstrated case management competency.
• Experience coordinating services for individuals with behavioral health, substance use disorder, or complex psychosocial needs.
• Working knowledge of ASAM Criteria, Stages of Change, person-centered care, and recovery-oriented principles.
• Strong knowledge of community resources, discharge planning, and care coordination.
• Ability to document accurately and timely within an electronic health record.
• Strong written, verbal, organizational, and interpersonal communication skills.
• Ability to effectively manage multiple clients, priorities, referrals, and deadlines.
• Ability to work collaboratively within a multidisciplinary treatment team.
• Current CPR/First Aid certification or ability to obtain certification within 30 days of hire.

Preferred

• Master’s degree in social work, counseling, psychology, or another related behavioral health field.
• Two or more years of experience in residential, detoxification, outpatient, or co-occurring substance use disorder treatment.
• West Virginia behavioral health licensure or certification applicable to the individual’s professional discipline.
• Substance use disorder credentialing such as ADC, AADC, or another applicable state-recognized certification.
• Training or demonstrated competency in motivational interviewing, trauma-informed care, crisis intervention, or other evidence-based behavioral health practices.

Work Conditions & Physical Requirements

• Work is primarily performed within a behavioral health and substance use disorder treatment environment.
• Regular interaction with individuals experiencing substance withdrawal, emotional distress, behavioral health symptoms, or crisis situations should be expected.
• Occasional travel to community partner locations, appointments, meetings, trainings, or other community-based activities may be required.
• Ability to sit, stand, and walk for extended periods.
• Ability to occasionally lift or move up to 25 pounds.
• Adequate vision and hearing to observe client needs and communicate effectively.
• Ability to concentrate, problem-solve, organize information, and manage competing priorities in a fast-paced treatment environment.
• Must maintain professionalism, sound judgment, empathy, and composure when supporting individuals with high-acuity or complex psychosocial needs.
