Nurse Practitioner (full-time: 32 - 40 hours/week)

Description We are looking for a new team member to join a busy community practice that specializes in comprehensive, general pediatric care. As a Nurse Practitioner, you’ll be an integral member of a forward-thinking clinical team dedicated to delivering exceptional patient care and service in a dynamic healthcare environment. Nurse Practitioner Essential Functions: Performing physical examinations of patients to evaluate health and diagnose illnesses Administering and performing diagnostic tests, such as x-rays and laboratory tests Creating patient care plans and contributing to existing ones. Detecting changes in patients' health and modifying treatment plans, as needed. Educating and counseling patients and families on presenting health problems, treatment plans, and home care Facilitating the patient’s transition within and between health care settings, e.g., admitting, transferring, and discharging patients. Collaborating with multidisciplinary team members by making appropriate referrals.
