PT - Physical Therapist

Home Health Physical Therapist

This is a 13-week contract position.

Day shift, 5x8 hours (08:00 - 17:00)

We are seeking a skilled and compassionate Home Health Physical Therapist to provide quality care to our clients in their homes. The ideal candidate will have experience in handling patients with cardiopulmonary, geriatrics, neuro, ortho, and oncology conditions.

Responsibilities

• Conduct assessments and evaluations of patients' physical therapy needs.
• Implement and monitor patient care plans, ensuring adherence to established protocols.
• Educate patients and families on proper exercise techniques and safety measures.
• Document patient progress and modify treatment plans as necessary.
• Perform home visits, focusing on the individual needs of each patient.

Required Experience / Certifications / Licensure

• Minimum of 2 years of home health experience required.
• Current state Physical Therapy license is mandatory.
• BLS certification is required.
• OASIS training is preferred.
• Valid driver's license and reliable transportation for home visits are necessary.
• COVID-19 vaccination is required, as well as a flu shot or declination with mandatory mask compliance at work.
