Job Description
Physical Therapist - Home Health

Schedule

Every 3rd weekend required.

Assignment Details

Home skilled visits - 6-8 per day, with a minimum of 6 points per day, at the manager's discretion may increase by 1 point.

Requirements

• 2 years of home health experience required.
• Valid state license.
• Certifications: BLS.
• Must have a valid driver's license and car for home visits.
• COVID vaccination required or exemption form on file.
• Flu shot required or declination form; mandatory mask wearing while at work.
• OASIS training preferred.
• Experience with various medical equipment and devices.

Responsibilities

• Conduct patient assessments focusing on cardiopulmonary, neurological, orthopedic, and oncology specialties.
• Develop and implement home exercise programs tailored to patient needs.
• Educate patients and their families on medication management and signs of physical, sexual, and psychological abuse.
• Document patient progress and care in required format.
• Promote safety and injury prevention strategies during home visits.
