Inpatient LVN Case Manager — Collaborative Care
SIERRA MEDICAL GROUP, INC
Heritage Sierra Medical Group in California is seeking an Inpatient Case Manager (LVN) to assess and coordinate care for patients during acute episodes. You will work with an interprofessional team to plan treatment, document progress, and facilitate transfers within established guidelines. The role requires an active LVN license in California and at least 1 year in case management.
