Social Worker Case Manager

Role Overview The Case Manager Social Worker (LMSW/LCSW) collaborates with the clinical team to coordinate patient discharge plans, aiming to enhance patient outcomes and ensure efficient healthcare utilization. This full-time position involves working three 12-hour shifts per week, from 7:00am to 7:30pm, including weekends. Key Responsibilities Conduct assessments to identify patient care needs and develop discharge plans, especially for high-risk patients. Work closely with healthcare providers to implement discharge strategies. Stay updated on healthcare trends, regulations, and payer requirements related to patient care and discharge planning. Qualifications & Skills Master's Degree in Social Work (MSW) required; LMSW certification necessary, LCSW preferred. Minimum of 2 years hospital social work experience, including post-graduate internship or related roles. Valid MD Social Work License appropriate to education level.
