Social Services - Medical Social Worker

Overview We are seeking a dedicated professional to join our team in Baltimore, MD. The position involves working shifts from 8:00 AM to 4:30 PM, five days a week, with weekend rotation every 7 weeks and holiday coverage as needed. There is no floating requirement for this position. Responsibilities Shift schedule: 8:00 AM to 4:30 PM, five days per week. Weekend rotation every 7 weeks. Holiday coverage as needed. No floating requirement for this position. Qualifications Acceptable candidates include RN Case Manager Discharge Planners or Licensed Social Workers. Social workers must be eligible for LMSW/LGSW/LCSW and must have passed the required exam. DC RN Licensure is required. Pending licensure with proof of application is acceptable, or the candidate can hold LMSW/LGSW/LCSW. BLS certification is required. Minimum of 2 years of experience. Acute hospital discharge planning experience. Required skills include experience in Emergency Department and Case Management. Flu vaccine required.
