Social Services - Medical Social Worker

Overview We are seeking a dedicated professional to join our team in Baltimore, MD. The position involves working shifts from 8:00 AM to 4:30 PM, five days a week, with weekend rotation every 7 weeks and holiday coverage as needed. There is no floating requirement for this position. Qualifications Acceptable candidates include RN Case Manager Discharge Planners or Licensed Social Workers. Social workers must be eligible for LMSW/LGSW/LCSW and must have passed the required exam. DC RN Licensure is required. Pending licensure with proof of application is acceptable, or the candidate can hold LMSW/LGSW/LCSW. BLS certification is required. Minimum of 2 years of experience. Acute hospital discharge planning experience. Required skills include experience in Emergency Department and Case Management. Flu vaccine required.
