Behavioral Health Services Coordinator

Under direct supervision, conducts psychosocial assessments and interviews with patients at Midtown Health Center to identify behavioral health needs, and appropriate referrals and resources. Connects patients to short- and long-term behavioral health services and supports.

Essential Duties and Responsibilities

1. Conducts psychosocial assessments and provides basic interventions using social work approaches including problem-solving, crisis intervention, task-centered, and solution-focused care. Develops safe, patient-centered care plans.
2. Connects patients with behavioral health services, community resources, and other supports. Follows up to monitor progress, confirm linkage to care, and assess whether resources meet patient needs.
3. Provides immediate and follow-up referrals to promote patient stability and continuity of care.
4. Consults with colleagues and interdisciplinary team members regarding assessment, care planning, interventions, and coordination of services.
5. Provides support, education, and advocacy to patients and families and encourages active participation in care.
6. Participates in interdisciplinary meetings and assists patients in navigating the healthcare system and accessing appropriate services.
7. Completes timely and accurate documentation in accordance with organizational and regulatory requirements. Maintains confidentiality and obtains releases of information (ROI) when required.
8. Performs other duties and responsibilities as assigned by the supervisor.

Work Experience

1. Bachelor's degree in Social Work (BSW) accredited by Council on Social Work Education (CSWE).
2. LBSW (Bachelor Social Worker) licensure from the Maryland Board of Social Work Examiners preferred.
3. Knowledge of behavioral health services offered throughout Baltimore area preferred.
4. Experience with therapy preferred.
