Outpatient Social Worker: Care Coordination & Resources

Mount Carmel Health System is expanding a walk-in/same-day clinic to support community needs as it transitions away from ED services. The Social Worker coordinates care, connects patients to community resources, and addresses behavioral health and social needs to ensure timely support and continuity of care in an outpatient setting. Requirements include a Bachelor degree in Social Work, current LSW license in Ohio, and at least 1 year in discharge planning within acute or ambulatory care. #J-18808-Ljbffr Trinity Health
