Hybrid Care Manager Social Worker - Medicaid Resources

COPC Brand is seeking a Care Manager / Social Worker in Columbus, OH to assess social, psychological, and environmental needs and connect patients with community resources. You will work with primary care providers, nurses, and care coordinators to deliver whole-person, patient-centered care, including end-of-life discussions when needed. Five years of clinical care management experience, an Ohio licensed Social Worker, a valid driver’s license, and knowledge of Medicaid are required or preferred. #J-18808-Ljbffr COPC Brand
