By guiding clinical staff and fostering a collaborative environment, this role empowers teams to grow and perform at their best directly enriching the patient experience.Through effective coordination, clear communication, and adherence to professional and regulatory standards, this leader drives a culture of excellence, safety, and trust shaping quality outcomes and making a meaningful difference in the lives of patients and the professionals who serve them.Patient Care Manager Key ResponsibilitiesProvide clinical supervision to ensure patient care meets professional standards, agency policies, and regulatory requirementsCoordinate and monitor patient care deliverySupervise clinical staff to ensure consistent, appropriate serviceMaintain clear communication with patients, caregivers, referral sources, and field and office staffDrive quality outcomes by meeting department goals and supporting improvement initiativesModel professionalism and organizational values in daily workUphold compliance through required training, accurate reporting, and cooperation with auditsPromote a safe work environment by following established safety protocolsSupport staff growth through coaching and continuing educationQualifications:Registered nurse with current licensure to practice nursing in the practicing state.Previous experience in home care setting with two years management or supervisory experience, preferred.Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirementsWhy is This a Great Opportunity:Strong job stability & demandSolid pay with benefitsMeaningful hands-on work
