Job Overview Medical Social Worker (MSW) – Home Health and Hospice Location: Elmhurst, IL, USA Employment type: Full‑time Hours: 40 per week Responsibilities Deliver skilled, patient‑centered care in a home health setting, in collaboration with an interdisciplinary team. Conduct comprehensive psychosocial assessments, identify patient and family needs, and develop individualized care plans that address emotional, social, and environmental factors. Educate and support patients and families by connecting them with community resources, counseling services, and care planning guidance to ensure they feel informed, supported, and empowered. Apply critical thinking and problem‑solving skills to navigate complex situations, advocate for patient needs, and maintain accurate, timely, and compliant EMR documentation. Work independently and travel to patient homes within the assigned territory. Qualifications Master’s in Social Work from a graduate school accredited by the Council of Social Work. One to two years post‑graduate social work experience in a health care setting. In good standing, a license as a Medical Social Worker issued by the state in which you work. Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.
