TLC Nursing Associates, Inc. is seeking a compassionate and dedicated Medical Social Worker to provide emotional support, counseling, and resource coordination to patients and families. The Medical Social Worker plays a key role in assisting individuals to cope with the social, emotional, and financial challenges associated with illness, hospitalization, or ongoing medical treatment.

Job Responsibilities

• Conduct psychosocial assessments to evaluate patient needs, strengths, and support systems.
• Develop individualized care plans addressing emotional, social, and environmental factors affecting patient well-being.
• Provide counseling and crisis intervention to patients and families coping with illness, loss, or life transitions.
• Assist patients and families in accessing community resources, financial assistance programs, and social services.
• Collaborate with physicians, nurses, and interdisciplinary team members to ensure holistic and coordinated care.
• Facilitate patient discharge planning, ensuring appropriate post-hospital care and support arrangements.
• Document all social work interventions, care coordination activities, and progress notes in the electronic health record (EHR).
• Advocate for patients' rights and ensure culturally sensitive care delivery.
• Participate in team meetings, case conferences, and quality improvement initiatives as needed.

Qualifications

• Master's degree in Social Work (MSW) from an accredited program required.
• Current state licensure as a Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) required, depending on state regulations.
• Minimum of 1 year of experience in a healthcare, hospital, or community health setting preferred.
• Knowledge of medical terminology, discharge planning, and community resources.
• Strong communication, advocacy, and problem-solving skills.
• Ability to provide compassionate, patient-centered support in sensitive or stressful situations.
