Empath Health's Trustbridge Hospice is seeking an RN Hospice Case Manager to join our Broward/Palm Beach County team. The role is PRN, 8:30am–5pm, with weekend availability and a focus on compassionate, patient-centered care. You will collaborate with the IDG to assess needs, develop care plans, and support families through advanced illness with dignity and comfort.
