Compassionate Hospice RN / Case Manager (Home Care)

Gentiva Hospice is seeking a dedicated Hospice RN / RN Case Manager to lead patient assessments, plan of care development, and education in home settings across Florida. You will coordinate with physicians, LPNs, CNAs, and social workers to deliver compassionate, high-quality end-of-life care.
