Field RN Case Manager /Fri-Sun (8 am - 8 pm)

Job Responsibilities Assess and identify patient/family needs, utilize nursing process, and coordinate Plan of Care with the Interdisciplinary Group (IDG). Provide palliative and supportive care to the patient/family unit. Make after‑hours visits to patients in their home, assisted‑living facility, nursing home, or hospital within the assigned territory. Assess patients/families and identify immediate needs; manage interventions by phone and/or during visits. Provide direct care as part of the IDG, incorporating psychosocial, spiritual, cultural, physical, and biological components. Demonstrate clinical skills for case management, application of company policy, physical assessment, IV therapies, pain medication titration, symptom management, and patient/family relationships. Prioritize, consult, refer, and collaborate with other professionals to promote quality and continuity of patient/family care. Educate patient, family, caregivers, and other health professionals about disease process, palliative interventions, caregiving, dying process, and safety practices. Report changes in patient condition to appropriate IDG members or health professionals; attend patient deaths, prepare body for transport, oversee medication disposal, and provide bereavement support. Complete documentation per policy and procedure; support SBAR communication; discuss needs with after‑hours staff. Perform duties during emergencies, reporting to the designated location as required. Perform other duties as assigned. Qualifications Current RN license in the state of employment. Minimum 1year nursing experience; hospice or hospital experience preferred. Previous experience with EMR/EHR system. Proficient in English (verbal and written); bilingual in Spanish preferred. Strong clinical and assessment skills; critical decision‑making and problem‑solving. Excellent time‑management, prioritization, and ability to handle stress professionally. Strong patient service and interpersonal skills. Computer proficiency: MS Office, data entry, online research. Available to work shifts to accommodate extended hours of operation. Ability to perform sedentary work with frequent interruptions and manage emotional stress when caring for terminally ill patients. Ability to meet essential physical requirements (lifting, bending, pushing/pulling, kneeling) with or without reasonable accommodations. Active Basic Life Support (BLS) certification from the American Heart Association or Red Cross. Compliance with drug and alcohol testing and Florida Care Provider Background Screening Clearinghouse requirements. Compensation Pay Range: $65,996.22 – $98,994.33.
