RN Case Manager — Home Hospice Care

Chapters Health in Florida is seeking an experienced Registered Nurse, Case Manager to assess patient and family needs, coordinate Plan of Care with the IDT, and provide clinical, palliative and supportive care to keep patients at home as long as possible. The role requires an active RN license, at least 1 year of nursing experience (hospice or hospital preferred), and an ability to travel to patient homes while using EMR/EHR systems.
