Home-Based Hospice RN Case Manager - Flexible Scheduling

St. Mary’s Home Care and Hospice in Bucks County, PA is seeking a full-time Hospice RN Case Manager to provide nursing care to hospice clients as prescribed by physicians and in alignment with state regulations. You will assess, plan, intervene, and supervise patient care and outcomes and collaborate with an interdisciplinary team. The RN will participate in weekly Interdisciplinary meetings and coordinate with the treating physician or hospice medical director to manage patient symptoms. #J-18808-Ljbffr Trinity Health
