$90,000 - $95,000 yearly

Responsibilities:

• Lead and coordinate patient care activities, ensuring each resident receives personalized, compassionate attention.
• Monitor and assess residents' health status, promptly addressing any changes or concerns to maintain optimal well-being.
• Administer medications and treatments accurately, following established protocols to support residents' health and recovery.
• Collaborate with a multidisciplinary team to develop and implement individualized care plans that enhance residents' quality of life.
• Educate residents and their families on health management, fostering understanding and encouraging active participation in care decisions.
• Maintain detailed and accurate medical records, ensuring compliance with healthcare regulations and standards.
• Participate in continuous professional development, embracing opportunities to expand your skills and knowledge in a supportive environment.

Qualifications:

• Experience in providing compassionate and personalized care to elderly residents.
• Ability to monitor and assess health status, addressing changes promptly and effectively.
• Proven track record of training and supervising staff in medication administration and treatment protocols.
• Strong communication skills to educate residents and families on health management and wellness.
• Proficiency in maintaining detailed and accurate medical records, ensuring regulatory compliance.
• Commitment to continuous professional development and staying current with nursing best practices.
• Experience in conducting psychotropic reviews and skin assessments, ensuring medication availability as prescribed.

VitaCare Living is a community-based residential facility (CBRF) dedicated to adding LIFE to the years of our residents and staff through compassionate, person-centered care. With 17 locations within Wisconsin, we care for over 300 residents in smaller, home-like settings that prioritize community, dignity, and purpose.
