CareNational is seeking a dedicated and compassionate Occupational Therapist (OT) for a full-time position with a respected Long-Term Care/Post-Acute Rehab facility in Roseburg, OR . This is a great opportunity to provide personalized therapy that supports residents in regaining independence and improving quality of life.

Pay Details:
$51.00 - $53.84 per hour (Depending on Experience)

Work Setting:
Onsite | Rehab Department - Long-Term Care / Post-Acute Facility

Position Details:
Location: Keizer, OR
Schedule: Monday through Friday
Shift: Variable Schedule
Hours: 40 Hours per Week
Start Date: ASAP

Certification Requirements:

• Occupational Therapist License - Oregon (Required)

• BLS Certification (Required)

Skill Requirements:

• Licensed to practice as an Occupational Therapist in Oregon

• Ability to assess, develop, and implement individualized therapy plans

• Strong communication and interpersonal collaboration with interdisciplinary teams

• Skilled in setting patient-centered goals and evaluating progress

• Ability to complete accurate and timely documentation

• Demonstrates critical thinking and effective problem-solving

• Experience in long-term care, post-acute, or rehab settings preferred

Key Responsibilities:

• Evaluate residents and determine therapeutic needs

• Develop treatment plans and obtain physician approval

• Deliver therapy according to approved plans

• Document evaluations, treatments, and progress notes accurately

• Participate in discharge planning and care transitions

• Screen residents for occupational therapy needs

• Collaborate with facility staff to promote optimal outcomes

• Ensure compliance with regulatory and facility standards
