CareNational is seeking a dedicated and compassionate Occupational Therapist (OT) for a full-time position with a respected Long-Term Care/Post-Acute Rehab facility in Roseburg, OR . This is a great opportunity to provide personalized therapy that supports residents in regaining independence and improving quality of life.

Pay Details:
$51.00 - $53.84 per hour (Depending on Experience)

Work Setting:
Onsite | Rehab Department - Long-Term Care / Post-Acute Facility

Position Details:
Location: Keizer, OR
Schedule: Monday through Friday
Shift: Variable Schedule
Hours: 40 Hours per Week
Start Date: ASAP

Certification Requirements:

Occupational Therapist License - Oregon (Required)
BLS Certification (Required)

Skill Requirements:

Licensed to practice as an Occupational Therapist in Oregon
Ability to assess, develop, and implement individualized therapy plans
Strong communication and interpersonal collaboration with interdisciplinary teams
Skilled in setting patient-centered goals and evaluating progress
Ability to complete accurate and timely documentation
Demonstrates critical thinking and effective problem-solving
Experience in long-term care, post-acute, or rehab settings preferred

Key Responsibilities:

Evaluate residents and determine therapeutic needs
Develop treatment plans and obtain physician approval
Deliver therapy according to approved plans
Document evaluations, treatments, and progress notes accurately
Participate in discharge planning and care transitions
Screen residents for occupational therapy needs
Collaborate with facility staff to promote optimal outcomes
Ensure compliance with regulatory and facility standards
