Start Date: ASAP Certification Requirements:
- Occupational Therapist License - Oregon (Required)
- BLS Certification (Required)
- Licensed to practice as an Occupational Therapist in Oregon
- Ability to assess, develop, and implement individualized therapy plans
- Strong communication and interpersonal collaboration with interdisciplinary teams
- Skilled in setting patient-centered goals and evaluating progress
- Ability to complete accurate and timely documentation
- Demonstrates critical thinking and effective problem-solving
- Experience in long-term care, post-acute, or rehab settings preferred
- Evaluate residents and determine therapeutic needs
- Develop treatment plans and obtain physician approval
- Deliver therapy according to approved plans
- Document evaluations, treatments, and progress notes accurately
- Participate in discharge planning and care transitions
- Screen residents for occupational therapy needs
- Collaborate with facility staff to promote optimal outcomes
- Ensure compliance with regulatory and facility standards
