Day Shift | Monday – Friday | 8:00 AM – 4:30 PM
A healthcare organization in Northern California is seeking an experienced Home Health Physical Therapist for a 13-week travel assignment in Clearlake, CA. This role focuses on providing in-home therapy services designed to restore mobility, improve function, and support patients in maintaining independence within their home environment.
This is an excellent opportunity for a therapist who enjoys working autonomously while collaborating with interdisciplinary care teams.
Position DetailsSchedule: Monday – Friday, 8:00 AM – 4:30 PM
Shift: Day shift (8 hours)
Contract Length: 13 weeks
Setting: Home Health
Mileage Reimbursement: Paid at the federal government rate
Weekend Opportunities: Optional shifts may be available
Perform comprehensive patient evaluations and develop individualized treatment plans
Deliver therapy interventions to restore and improve functional mobility and independence
Utilize appropriate clinical techniques and treatment protocols
Establish care plans and discharge plans while coordinating with interdisciplinary teams
Document patient care and progress accurately and within required timeframes
Educate patients and caregivers on therapy plans and home exercises
Supervise therapy assistants when assigned
Physical Therapist license (active and in good standing)
Home Health experience required
HCHB (Homecare Homebase) experience required
OASIS documentation experience required
Minimum 2 years of Physical Therapy experience preferred
BLS or CPR certification required
Valid driver’s license and reliable transportation required
