Local Travel Home Health Physical Therapist
Local travel home health physical therapist provides comprehensive patient evaluations and individualized treatment plans in the home setting, ensuring mobility and independence. This 13-week contract offers competitive weekly compensation and the chance to impact patient outcomes across the region.
Key Responsibilities:
- Conduct comprehensive patient evaluations in home settings
- Develop individualized treatment plans to improve mobility, strength, balance, and independence
- Perform start-of-care assessments, recertifications, and routine visits
- Complete accurate OASIS documentation and maintain compliance with regulations
- Serve as case manager coordinating care with physicians, nurses, therapists, and interdisciplinary teams
- Educate patients and caregivers on therapeutic exercises, mobility strategies, and safety precautions
- Monitor patient progress and modify treatment plans as needed
- Accurately document all patient interactions and outcomes using Matrix EMR
- Deliver compassionate, patient-centered care while promoting functional independence and quality of life
Qualifications:
- Doctorate or Master's degree in Physical Therapy from an accredited program
- Active Missouri Physical Therapist license
- Strong experience with OASIS documentation, case management, start-of-care assessments, and recertifications
- Excellent communication, organizational, and time-management skills
- Ability to work independently in a home health environment
Preferred Skills:
- Current CPR/BLS certification
- Home health experience for travel assignments
- Experience with Matrix EMR
