Uses knowledge of available resources to ensure patient care needs are met and transitions of care coordinated.Minimum QualificationsMaster's Degree in counseling, social work, or other clinical discipline, such as psychology.2 years experience in counseling or social work in a mental health related field.Ability to use good judgement to respond to emergencies.Ability to establish program goals and objectives that support the strategic plan.Comprehensive understanding of behavioral health services and appropriate levels of care.Familiarity with differential diagnosis of common mental health and/or substance use disorders.Knowledge of public health problems, principles, and practices.LCSW - Licensed Clinical Social Worker - State of Pennsylvania Upon Hire orLPC - Licensed Professional Counselor - State of Pennsylvania Upon HirePhysical Demands Lift and carry 7 lbs., continuous sitting >67%, frequent keyboard use/repetitive motion, frequent fine motor activity/wrist position deviation.Job Description Disclaimer: This position description provides the major duties/responsibilities, requirements and working conditions for the position.
