Nurse Practitioner for In-home Visits

Overview Board-certified nurse practitioner contractors required to conduct In‑home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members at Signify Health. Responsibilities Perform in‑home visits focusing on overall health and well‑being and identifying potential barriers to care. Conduct medication and medical history review, and a physical evaluation in the member’s home. Use pre‑populated, patient‑specific forms for each assessment. Do not prescribe medicine, order lab tests, or alter current treatment regimens. Qualifications Board‑certified nurse practitioner with a valid state license. Preferred: Medicaid license or willingness to enroll. Independent contractor, 1099 status. Compensation & Schedule Pay range: $80 per completed assessment; average 6‑9 visits daily. Potential for up to 135% rate increase via Market Multiplier. Additional pay via pay‑per‑test model for qualifying members. Shifts available Monday‑Sunday (no nights or weekends required), short‑term or long‑term basis. Airfare, hotel, and rental car provided as eligible. Malpractice coverage; availability incentives based on weekly availability. Flexible schedule. #J-18808-Ljbffr
